What is a blepharoplasty?

Blepharoplasty is an operation that removes excess skin from the upper eyelids. The excess skin is usually due to Blepharoplasty is an eyelid surgery that removes excess skin, muscle and/or fat from around the eyes. As you age, the skin around your eyes can stretch and your muscles can weaken, causing excess fat to appear on both the upper and lower eyelids. 

Upper and lower blepharoplasties are both available for people looking to remedy this.

An upper blepharoplasty

An upper blepharoplasty is typically performed if you are concerned about ‘heavy-feeling’ upper eyelids. You may have noticed a loss in the elasticity in the skin, causing a more hooded appearance above your eyes. 

This eyelid surgery can be carried out for the cosmetic purpose of creating a more alert, awake appearance or for the functional purpose of improving your vision by removing overhanging skin.

A lower blepharoplasty

A lower blepharoplasty is most often done for cosmetic purposes. You’d typically get this procedure when concerned about deep creases under the eye, eye bags and dark circles. Again, these eyelid surgeries are carried out so that you can achieve a more alert appearance.

How is a blepharoplasty performed?

Blepharoplasties are usually outpatient surgeries. Beforehand, the surgeon will measure and mark the areas where skin will be removed. Anaesthesia drops are used to numb the eyes. The face is then cleansed using an antiseptic iodine solution and covered by drapes.

Anaesthetic is injected under the eyelid skin. This can be slightly painful however it means you shouldn’t be able to feel anything during the procedure.

Excess skin is removed using a fine scalpel and electrocautery, and excess fat is repositioned or removed. The wounds are then closed with sutures. After the procedure, ointment is put on the wounds and eye pads are applied to the eyelids.

Transconjunctival blepharoplasties

There are a number of techniques that can be used during blepharoplasties. Blepharoplasties can also be combined with other procedures such as brow lifts and skin resurfacing. 

A transconjunctival blepharoplasty is one of the techniques that can be used to improve the appearance of the lower eyelids. This is where an incision is made inside the lower eyelids. This particular procedure can reduce the appearance of scars, reduce recovery time and create a more natural appearance.

How to prepare for a blepharoplasty

Before a blepharoplasty, you will always have to meet with your surgeon for a preoperative consultation.  Here, you will discuss your medical and surgical history, your medication and your expectations from the surgery. It may also be recommended that you meet with an eye specialist. They will be able to determine the right steps for you to take before the surgery. 

You will also undergo a physical examination, vision examination and your eyes will be photographed.

What happens after a blepharoplasty?

After eyelid surgery, your eyes are covered with dressings. The wounds are treated with antibiotic ointment and you are provided with cool packs. Following a period of monitoring, you typically have the ability to return home on the same day. 

Your surgeon will provide you with a post operative plan, which can include using ice packs, prescribed eye drops or ointments. You will also be asked to avoid strenuous activities, swimming, smoking, rubbing your eyes as well as contact lenses for the first few weeks. Dark sunglasses will be recommended, and sleep tips will also be given.

After a blepharoplasty, you may experience bruising, swelling, blurred vision, watering eyes and double vision. You may also be sensitive to light and have some pain or discomfort. Bruising and swelling can last for between two to four weeks, and scars can take a few months to fade.
