    appreciated the opportunity to see Professor Chinoy and I recognise his substantial expertise in rheumatology and inflammatory muscle conditions. He was polite, clear, and reassuring in his manner. However, I left the appointment feeling that some key parts of my longitudinal history were not fully integrated into the assessment. Despite having uploaded all my scans from 2021, 2022, and September 2025, these were not able to be reviewed together during the consultation because the files were not opened on the system. This meant we were unable to discuss the progression pattern across the years — including the shift from right-sided abnormalities (fatty infiltration in 2021, right psoas sheath oedema in 2022, and right paraspinal inflammation in May 2025) to the newly emergent left-sided oedema in September 2025. This remains one of my main concerns, and I had hoped to go through it as a connected clinical story. I also felt somewhat discouraged when the peer-reviewed publications I mentioned — particularly the Brain journal review “Axial Myopathy: an overlooked feature of muscle diseases” — were dismissed as unhelpful for patients to read. I fully understand the concern about online searches, but the review is a major academic paper describing: • axial myopathy being frequently misattributed to spinal degeneration, • seronegative inflammatory myopathies, • cases where paraspinal involvement is the earliest or only manifestation, and • the consistent issue that MRI abnormalities in the paraspinal muscles are under-recognised by non-neuromuscular specialists. These points felt directly relevant to my pattern of findings, particularly as my bloods have remained seronegative and several consultants have attributed the MRI abnormalities mainly to spinal wear-and-tear — something the paper specifically warns can lead to under-diagnosis. I appreciate that Professor Chinoy believes the fatty infiltration and oedema are most likely secondary to spinal changes. However, this does not fully account for: • the right-to-left progression seen on imaging, • the severe nocturnal pain and inability to turn in bed, • the sudden onset bilateral hand pain, stiffness and locking, • the persistence of symptoms despite minimal radiological change in the spine over four years, and • the coexistence of Hashimoto’s disease, which rheumatology felt was irrelevant despite autoimmune diseases sometimes clustering. I am grateful that he has arranged for radiology to re-review the entire MRI set, and I appreciate this very much. However, the consultation left me feeling that many parts of my history and symptoms remain unexplained, and that the published evidence on axial/paraspinal muscle disease — especially in seronegative patients — was not fully considered.

    I had two visits - a pre-surgical assessment and 3 days later I returned for breast cancer surgery with an overnight stay. I have a few hopefully constructive observations: There was no evidence that any information collected at the pre-op meeting was transferred to the team admitting me for my operation. The inefficiencies of this became laughable at times and is a waste of your precious resources and my time . Eg wanting to repeat mrsa tests and weight/height checks because they hadn’t checked their records from 3 days prior. I was told I couldn’t go down to surgery without a Pregnancy test. I had to be very firm that I had the right to refuse and was made to feel like I was being difficult when the truth was very different. The consultants stepped in to support my position and my right to self advocate. The number of different contact points with junior/assistant nursing staff during my 28hr stay was startling, I lost count. There was no continuity at all which led to more observations that necessary (literally 5mins apart because of a lack of communication) and at other times several hours without any contact at all I twice had to tell staff not to put blood pressure sleeve on the arm that just been operated on as they were clueless regarding the surgery I’d had. It’s a general ward so I accept there are no/fewer specialists (eg for breast cancer) but the lack of interest/knowledge was frustrating and in stark contrast to other private hospital experiences I’ve had during this journey. Minor point but there were no towels or toiletries in the en suite and the shower set up was totally impractical for post surgery washing (as the hand held show was missing). I was offered option to use en suite in another room - staff did their best to find a solution and my husband was able to support me.

    I had booked an echocardiogram with a female technician, and had taken the day off work to attend, but when I arrived and asked about the female technician (to confirm), I was told that a female was not available, which was extremely disappointing. I had called twice earlier in the day (before travelling to the appointment) to confirm that a female was available, but no one in the echo department answered the phone. Further, I have been attending the Cleveland Clinic since March of last year, when I had a subarachnoid haemorrhage, and had been told that my standing request for a female echocardiogram technician had been recorded in my patient profile. The staff in the imaging department seemed to be surprised that a woman having this test would insist on a female technician, which indicated a lack of sensitivity and training, and they appeared not to know how to handle this situation. So, not only did I waste my time and lose a work day due to this oversight, but I had to cancel my follow-up cardiology consultation, and now cannot take time off work to reschedule until the new year, which has created a very stressful situation for me. In my opinion, the Cleveland Clinic should always have a female echocardiogram technician on duty, for women who are uncomfortable with a male, and the front desk staff needs much better training in this regard.

    Very unorganised, got the bill twice with no explanation. Called and had to speak with 3 people and still no one knew what bill was correct. The Surgent secretary told me to ignore one of the invoices which turned out was wrong. First time I arrived for the consultation I was told that the surgent who I was going to see wasn’t there and they advised me to go home. It turned out he was there and waiting for me. Second time they couldn’t find my file and didn’t know what to do or why I was there. 3rd time lucky you would think, I was told to wait in the waiting room and I did without being told there were two waiting rooms and I was in the wrong room and the surgent called me and wondered if I was on my way. On the positive the surgent and the operation was great but I do wish it was a bit more organised and that the information given to the patients was with a follow up to everyone involved.

    Received urgent call requesting blood test for tolerance to Dye. I Moved appointments for that afternoon to facilitate a flying visit. After arrival kept waiting for 35 minutes until I asked reception as was needed elsewhere and was told phlebotomist would be with me shortly. With Phlebotomist approx 3 minutes, total time 50 minutes and 50 miles traveled Then had to wait 9 days for scan which was only scheduled following chase-up from private GP. On presenting for CT Scan was told that they did not require blood test as procedure did not indicate use of dye. Most unimpressive communication and administration throughout

    Had breast augmentation 22yrs ago and they needed replacing as was far too big for my body , so I really needed quite a bit of reshaping as my skin had got baggy and heavy around the implants , mr Hodgins suggested and new implant and a lift and quit a bit of skin removal.     It would really help to recieve more accurate information on the assessment procedure before attending. Details such as if parent remains with child, if appointment also needs a seperate parent meeting and an indicator of when diagnosis would be made. It didn’t really tie up with pdf sent on welcome email. Would be better to schedule in parent appointment at a similar time as first child assessment appointment, or/ and make it clear this is the procedure. Phoning up Harley mind care prior to attending did not help me to understand & actually conflicted with what happened at appointment.
