Mr Paul Modi is one of a small number of surgeons in the UK performing minimally invasive keyhole heart surgery and is currently the only surgeon in the UK undertaking robotic mitral valve surgery and robotic coronary artery bypass grafting. He is the only surgeon in the UK to perform Totally Endoscopic Robotic Mitral Valve Repair, and patients travel from across the world to seek his expertise. He is also a national and international trainer in these advanced techniques.
Mr Modi trained in the leading North American and German heart centres under all of the original pioneers of minimally invasive mitral valve surgery more than 20 years ago. He spent 12 months in the United States as a Clinical Instructor in Robotic Heart Surgery, working alongside Dr Randolph Chitwood Jr at the East Carolina Heart Institute. There, he developed advanced skills in minimally invasive and robotic cardiac valve surgery and atrial fibrillation surgery. During this time, he recognised the significant benefits of keyhole heart surgery, particularly the faster recovery compared to conventional open heart surgery, where the breastbone is divided.
He was subsequently awarded two scholarships, including the prestigious Society Scholarship for Cardiac Surgery, to train with the European pioneers of minimally invasive cardiac surgery: Professor Fred Mohr in Leipzig, Germany, and Professor Hugo Vanermen in Aalst, Belgium. He undertook further super-specialist training in minimally invasive valve and coronary surgery (including MIDCAB procedures) and thoracoscopic atrial fibrillation surgery across Germany, Belgium and The Netherlands.
On returning to the UK, Mr Modi introduced many of these pioneering techniques into his UK practice. He is a Consultant Cardiac Surgeon at Liverpool Heart and Chest Hospital NHS Foundation Trust, the largest standalone specialist cardiac and thoracic surgery centre in the UK, where over 2,100 open-heart operations are performed each year.
His philosophy is to deliver the same level of surgical excellence seen in the major North American and German centres such as the Cleveland Clinic and Mayo Clinic. In addition to his clinical work, Mr Modi teaches surgical skills and basic sciences at the Royal College of Surgeons of England and trains fellow consultants in advanced surgical techniques. He practises both within the NHS and the private sector.
Mr Modi has appeared live on the BBC’s The One Show, which featured him performing a keyhole mitral valve repair; the patient returned to playing golf just three weeks later. He has edited five surgical textbooks and authored more than 70 peer-reviewed publications, many focusing on the benefits and outcomes of robotic and minimally invasive cardiac surgery.

My journey started with what should have been a routine Angiogram, I was then told that it wouldn’t be sufficient to complete my arterial needs as there where to many blockages. To which I was told the only option would be open heart surgery to complete a bypass. At 43 to be given this news was Petrifying. My life felt like it wasn’t my own anymore and was in unknown waters. Not knowing which way to turn I was then referred to Paul Modi at the Spire and what seemed to me like a major risk was explained so calmly and professionally, seemingly I only needed “a bit of fancy plumbing” he reassured and put me and my family at such ease that going forward with the operation I would be safe hands. Paul’s Secretary Jodi had me booked in within 4 weeks and was very informative and sympathetic with my emotional support before theatre. When the day came of my surgery, the realisation and magnitude of what lay ahead of me became very daunting, but once again Mr Modi went through all possible scenarios and options of what to expect and once again reassured me with his fantastic personal bedside manner and confidence. It’s a couple of weeks since my minimally invasive mitral valve repair and I’m feeling so much better. My care throughout my stay at Spire, Didsbury was excellent. I felt supported the whole time from arrival to discharge. Mr Modi performed an amazing piece of surgery to repair my mitral valve beautifully and his care and attention before and after the surgery were above and beyond expectations. My anaesthetist Dr Lee Feddy also gave great support and instilled confidence despite a daunting procedure and checked up on me regularly post-op. The nursing team and doctors in CCU and after on ward were superb; compassionate, caring and attentive to my needs. They made a huge difference to my post-op experience and recovery, thank you. This was a very very complex mitral valve repair which I suspect no other surgeon in the UK would have attempted, but Mr Modi was determined to give it a try to get the best possible outcome for me. From the beginning he gives you confidence in both himself and his team and the care has been excellent from start to finish, not just for the surgery itself but also the follow up - Mr Modi hasn’t missed a day to come and see me and explain exactly what’s going on. I honestly can’t praise or thank him enough for his skills and care, which have been beyond my expectations.
From the first time I met Mr Modi, I had complete confidence in him. He was extremely knowledgeable and gave very clear explanations of the risks and benefits of surgery. I had a minimally invasive LIMA CABG procedure by Mr Modi and his team at Manchester Spire. Preoperative care was very good with clear instructions on how best to prepare for the admission. Mr Modi counselled very well and I felt my consent was well informed. Postoperative care of my family I am especially grateful for: my wife received a timely call from Mr Modi. Whilst on critical care unit, Mr Modi responded promptly to the night doctor’s query and then reviewed me in the morning, explained to me the key findings, and made clear instructions to the staff. The next day, he attended me on the ward just after 7am, clinically made a diagnosis of a complication responsible for some extra pain I had, which then responded promptly to specific treatment. He then left for abroad but remained in touch daily with me and the ward staff, directing treatment and discharge. Following discharge home on day 4 post op, I received initially daily enquiries from Mr Modi on my progress and he gave timely clear instructions on how to deal with additional issues that developed whilst I was at home by promptly responding to my messages. His advice proved to be completely correct. On reflection, throughout the experience, I felt in safe hands and my family were confident that it was so indeed. After several tests ordered by my cardiologist Dr Kalla and professor Steeds, in Birmingham , it became evident that I had severe mitral valve regurgitation. They both suggested that I should get the valve fixed between 6 months to two years and suggested I have a consultation with Mr Modi in Manchester. Mr Modi is one of the few heart surgeons who performs many minimal invasive heart procedures including robotic every year. I first met Mr Modi in September last year. Mr Modi explained everything in detail including options and outcomes to me clearly. He instantly made me feel at ease and confident about the procedure. I underwent minimally invasive mitral valve repair, with PFO closure, under his care in July 2026. From the very beginning, the whole process was made smooth and reassuring. The pre-operative communication and preparation were excellent, which made what could have been a daunting experience feel calm and well managed.
