Mr Navaratne completed a Major Trauma fellowship at the Level 1 Major Trauma Centre at St Mary’s Hospital. He subsequently undertook travelling fellowships to Kingston, Jamaica for the management of complex abdominal injuries with a focus on gunshot trauma and the David Dunn Fellowship to Hospital San Carlos in Madrid for Upper GI Surgery. Mr Navaratne is a Fellow of the Royal College of Surgeons.
Mr Navaratne is highly specialised in the management of abdominal wall conditions including diastasis recti and all types of hernia. His expertise span minimally invasive (keyhole) hernia and diastasis recti repair through to major abdominal wall reconstruction. Mr Navaratne is co-chair of the Imperial College Healthcare NHS Trust Abdominal Wall Multi-Disciplinary Team (MDT) which manages patients with complex abdominal wall conditions. Mr Navaratne’s patient-centred approach to treating disorders of the abdominal wall is always tailored to his patients needs in order to give them the best possible experience and outcome.
Mr Navaratne is an expert in the laparoscopic management of gallstones and its associated complications such as gallstone related pain (biliary colic), gallbladder infection/inflammation (acute cholecystitis), gallstones that have migrated into the common bile duct (common bile duct stones / choledocholithiasis), obstructive jaundice and gallstone pancreatitis. Mr Navaratne performs elective and emergency cholecystectomy (key-hole surgery for gallbladder removal) with or without intra-operative cholangiogram (to ensure the bile duct is clear of stones) and laparoscopic common bile duct exploration (key-hole surgery to remove stones from the bile duct). Mr Navaratne also has a special interest in the management of gastro-oesophageal reflux disease (GORD) including laparoscopic hiatus hernia repair and laparoscopic anti-reflux surgery (including fundoplication and insertion of the LINX device).
Mr Navaratne has published over 60 peer review papers in high impact medical journals. He has written several book chapters and is the author of the only textbook dedicated to Laparoscopic Common Bile Duct Exploration (LCBDE). He has presented his work at numerous international meetings and has received several awards/prizes for his research including the David Dunn Medal from the Association of Laparoscopic Surgeons of Great Britain and Ireland (ALSGBI).

Excellent diagnosis and advice on the correct procedure to fix my incisional abdominal hernia. Mr. Navaratne recently performed laparoscopic (keyhole) hernia surgery. During our initial meeting he informed me that the procedure involves three tiny cuts in the stomach and is minimally invasive. The consultation was never hurried and I was impressed with Mr. Navaratne’s knowledge and communication skills. The surgery went very well. I experienced almost no post-operative discomfort. We met 2 weeks after the surgery and by then the healing process was well under way. We had an enjoyable conversation during which Mr. Navaratne offered further insight into the procedure.

Doctor Lalin Navaratne went out of his way with care with professional and genuine feeling to bring in help right from first personal contact to discharging and beyond. He diagnosed well and communicated right the way through and undertook emergency surgery later in the day knowing it was going to be a complicated medical emergency he was confronted with.

This book provides a concise overview of several critically important techniques and their applications within Laparoscopic Common Bile Duct Exploration (LCBDE). LCBDE is rapidly emerging as the preferential management option for treating patients with common bile duct (CBD) stones. Chapters within this book describe how this technique can be applied to manage patients with concomitant gallstones, which aims to increase the exploration of the CBD via the transcystic route. Particular mention is made for the use of energy devices to enable stone fragmentation within the bile duct also known as Lithotripsy-Assisted Bile duct Exploration by Laparoendoscopy (LABEL). Guidance is also provided on how to set-up an acute biliary service which can offer the single-stage management of concomitant gallstones and choledocholithiasis. The concepts of ‘Leveraging Access to Technology and Enhanced Surgical Techniques (LATEST)’ in LCBDE and ‘Biliary Surgery 2.0’ as the framework through whichthese techniques can be further enhanced are analysed. Laparoscopic Common Bile Duct Exploration features insight into how to perform these increasingly important procedures successfully and set-up a service within institutions that are yet to provide it. Therefore, it is a valuable resource for all medical professionals involved in the care of these patients.

Background Many studies have failed to demonstrate significant differences between single- and two-staged approaches for the management of choledocholithiasis with concomitant gallstones in terms of post-operative morbidity. However, none of these studies paid specific attention to the differences between the methods of accessing the bile duct during laparoscopy. The aim of this study was to report outcomes of transcystic versus transductal laparoscopic common bile duct exploration (LCBDE) from our experience of over four hundred cases. Methods Retrospective review of 416 consecutive patients who underwent LCBDE at a single-centre between 1998 and 2018 was performed. Data collected included pre-operative demographic information, medical co-morbidity, pre-operative investigations, intra-operative findings (including negative choledochoscopy rates, use of holmium laser lithotripsy and operative time) and post-operative outcomes. Results Transductal LCBDE via choledochotomy was achieved in 242 patients (58.2%), whereas 174 patients (41.8%) underwent transcystic LCBDE. Stone clearance rates, conversion to open surgery and mortality were similar between the two groups. Overall morbidity as well as minor and major post-operative complications were significantly higher in the transductal group. The main surgery-related complications were bile leak (5.8% vs 1.1%, p = 0.0181) and pancreatitis (7.4% vs 0.6%, p = 0.0005). Median length of post-operative stay was also significantly greater in the transductal group. Conclusion This study represents the largest single study to date comparing outcomes from transcystic and transductal LCBDE. Where possible, the transcystic route should be used for LCBDE and this approach can be augmented with various techniques to increase successful stone clearance and reduce the need for choledochotomy.
