In an open rhinoplasty, a small incision is made across the columella between the nostrils, allowing the skin to be carefully lifted and giving the surgeon direct access to the underlying cartilage and bone. This can be particularly useful for complex nasal reshaping, tip surgery, structural reconstruction and revision rhinoplasty. In a closed rhinoplasty, all incisions are made inside the nostrils, meaning there is no external incision. This approach may be suitable for selected patients depending on the changes required and the anatomy of the nose. Neither approach is automatically better.
