The Doctor should be made aware of the patient's reason for the visit prior to the actual visit, as opposed to the patient handing him the Pre-Registration form.

My appointment was on time and I was welcomed by Dr Michael Rose and he asked me how i have been and I explained the tablets he prescribed were a big help and he upted the tablets strength 20 mg and then to 30 mg in a few days time which i am happy to try I asked Dr Rose a few questions about the cause of my problem and was it from lifting items too heavy and I was happy enough with his explanation
