Is It Possible to Achieve Favorable Accelerated Dental Changes with No Periodontal Complications When Retracting Upper Anterior Teeth Assisted by Flapless Corticotomy Compared to Traditional Corticotomy? A Two-Arm Randomized Controlled Trial

An orthodontic traction of a partially impacted upper canine. A 15-year-old female patient came with her mother for an orthodontic consultation. The chief complaint was the high position of the upper left canine.The clinical examination showed:1- Diastema between the upper central incisors.2- Upper midline shift to the left side.3- No enough space for tooth #23.4- Class 2 on the left side, Class 1 on the right side.So, the treatment plan was:Leveling and alignment for both dental arches with the use of open coil to create a good space for the left canine, close the diastema, and correct the upper midline shift.Then, I asked the patient to use class 2 elastics on the left side + box elastics on the right side + I did IPR on the lower anterior teeth in order to correct the class 2 relationship on the left side.Treatment duration is 1.5 years.Enjoy watching 🍿سحب تقويمي لناب علوي منطمر جزئيًا.جاءت مريضة تبلغ من العمر 15 سنة برفقة والدتها للاستشارة التقويمية. الشكوى الرئيسية كانت الموقع المرتفع للناب العلوي الأيسر.الفحص السريري أظهر ما يلي: 1. وجود فراغ (diastema) بين القواطع العلوية المركزية. 2. انحراف الخط الأوسط العلوي نحو الجهة اليسرى. 3. لا يوجد فراغ كافٍ للسن رقم 23 (الناب الأيسر العلوي). 4. علاقة سنية من الصنف الثاني في الجهة اليسرى، ومن الصنف الأول في الجهة اليمنى.لذلك، كانت خطة العلاج:رصف وتسوية للأقواس السنية العلوية والسفلية، مع استخدام نابض فتح مسافة (open coil) لتوفير فراغ كافٍ للناب الأيسر، وإغلاق الفراغ بين القواطع، وتصحيح انحراف الخط الأوسط العلوي.ثم، طلبت من المريضة استخدام مطاط الصنف ٢ (class 2 elastics) في الجهة اليسرى، ومطاط صنف اول بشكل مربع (box elastics) في الجهة اليمنى، وقمت بعمل سحل بسيط بين الأسنان الأمامية السفلية (IPR) بهدف تصحيح علاقة الصنف الثاني في الجهة اليسرى.مدة العلاج: سنة ونصف.استمتعوا بالمشاهدة 🍿"‎
