After junior appointments at University College Hospital, London; University Medical Centre, Nottingham; and the Royal Marsden Hospital, Dr Lever was appointed Registrar and then Senior Registrar on the Professorial Medical Unit, Endocrinology Diabetes & General Medicine at King's College Hospital, London. Between 1981 and 1983 he undertook a Research Fellowship in Endocrinology & Diabetes at the University of Chicago working in thyroid and diabetes research.
