I have been a practising doctor for a quarter of a century. It’s hard for me to believe that’s true and that amount has time has passed so quickly. I’ve seen huge changes in GP services during this time, and as I enter the latter years of my career, it makes me reflect on the value of a regular family doctor.

Over the years, the role of the family doctor has changed a lot. When I was a young, aspiring doctor, everyone knew their family doctor. Mine was Dr Wheldon. He was a lovely man and the family embraced him as a family friend. Come rain or shine, when we had a medical issue, we would call him and he’d be there. 

In those days, GPs had far fewer patients and had more time for the patients that needed their care. 

As time has moved on, this patient-doctor relationship has become fractured and now most people don’t even know who their doctor is, or whether they are even seeing a doctor.

Self-collect blood tests, pharmacy advice, online appointments can all be very helpful, but they can lack the essential element of a doctor’s knowledge. The one which allows the test results to be interpreted knowing your patient’s medical history. Often these tests will flag small issues, but a doctor’s experience and in-depth knowledge of the patient will allow them to know what the next steps are. Hence, the doctor-patient relationship is vital for a successful partnership when dealing with health issues. 

Knowing your patient, knowing how they react, what they do, how to communicate with them, and earning their trust are all essential skills of being a doctor.

Like any relationship, trust is a fundamental value that needs to be earned and nurtured. It can be easily ruptured, and this will only lead to poor communication and decision making which is not in a patient’s interest. 

Your doctor should help to guide you through your heath journey with their experience and advice. They can really make a difference to your long-term health outcomes, and you should not underestimate how valuable their lifetime of experience and skills is. This is why, more than ever, it is important to find yourself a good doctor who can invest their time in you. 

Why should you have a regular family doctor?

Fatigue is a **common issue** for many women and it’s more than just feeling tired after a long day. Ongoing fatigue can impact your life in many ways, and it’s important to understand the possible causes. A 2023 review of over 90 studies involving 600,000 participants found that **more than 18% of women experience some type of fatigue**. If you feel constantly drained then it could be worth investigating it.

## **What Causes Fatigue in Women?**

Fatigue can stem from a variety of sources, including medical conditions, hormonal changes, nutritional deficiencies, and lifestyle factors.

### 1. Medical Conditions

* **Chronic Fatigue Syndrome (CFS/ME):** A serious illness causing extreme tiredness, brain fog, and exhaustion. It’s often linked to recovering from viral infections like COVID-19 or Epstein-Barr virus (EBV).
* **Thyroid Issues:** Both an underactive (**hypothyroidism**) and overactive thyroid (**hyperthyroidism**) can cause fatigue. A simple blood test can determine if your thyroid is the issue.
* **Heart Disease:** Fatigue is a common but often overlooked symptom of heart disease, especially in women. If you’re in middle age and feeling unusually tired, some blood tests an an assessment by a doctor can determine if you need to see a cardiologist.
* **Sleep Disorders:** Conditions like sleep apnoea, insomnia, and restless leg syndrome can disrupt your sleep, leaving you feeling exhausted during the day. Sleep apnoea is especially common among women, so if you’re snoring or gasping for air at night, get checked out.

### 2. Hormonal Imbalances

Hormonal changes throughout life can affect energy levels, particularly:

* **Menstrual Cycle:** Fatigue can be more noticeable before your period, in the luteal phase, when hormone levels fluctuate. Premenstrual syndrome (**PMS**) and its severe form, **PMDD**, are also linked to low energy.
* **PCOS:** Women with polycystic ovary syndrome are more prone to fatigue, often due to poor sleep or insulin resistance.
* **Perimenopause and Menopause:** As oestrogen levels drop, many women experience fatigue, disrupted sleep, and night sweats.

### 3. Blood tests and Nutritional Deficiencies

Lack of certain vitamins and minerals can leave you feeling drained:

* **Baseline bloods:** Looking at kidney function, Thyroid function **HbA1c** (long term sugar), Liver and **full blood count** are all essential.
* **Iron Deficiency:** Low iron levels, especially in women with heavy periods, can cause **anaemia**, making you feel weak and fatigued.
* **Vitamin D Deficiency:** A lack of this “sunshine vitamin” can contribute to low energy, especially in those who don’t get enough sunlight.
* **Vitamin B12:** This vitamin is essential for energy. People following vegan diets or those with nutrient absorption issues may be at risk for deficiency.
* **Magnesium:** Nearly half of adults don’t get enough magnesium, which is important for many body functions, including energy production.

### 4. Mental Health

Mental health plays a significant role in fatigue. Conditions like **depression, anxiety, and chronic stress** are strongly linked to low energy levels. Feeling **“wired but tired”** is a common experience in women dealing with stress or anxiety.

### 5. Lifestyle Factors

Your habits can greatly influence your energy levels:

* **Poor Sleep:** Quality sleep is essential, especially during menopause. Aim for **6-9 hours** of good sleep each night.
* **Lack of Exercise:** Regular movement boosts energy and helps combat fatigue. Try to fit in **150 minutes of moderate exercise** per week.
* **Alcohol and Smoking:** Both smoking and excessive alcohol can drain your energy.
* **Diet:** Improving your nutrition and avoiding poor quality foods can help to boost energy levels. Avoiding **Caffeine and too much sugar** as well.

## **How to Boost Your Energy**

If you’re feeling fatigued, here are steps you can take to improve your energy levels:

1.  Get checked for any underlying health conditions.
2.  Prioritise good sleep.
3.  Eat a balanced diet rich in vitamins and minerals.
4.  Exercise regularly, even if it’s just small movements throughout the day.
5.  Manage stress and practice self-care.

If your fatigue is **persistent or unexplained**, talk to your doctor. Addressing the root cause can help you get back to feeling your best.

Tiredness – a common complaint from women

A new year is the time for renewing our goals and this often leads to a new health drive. It has never been so important to reflect on our health and maybe be kinder to ourselves but setting realistic goals that lead to permanent lifestyle changes is the ambition – not short-term fixes. 

In this era of social media and constant news reporting, it is increasingly complex for patients to know the right things to do. Constant, often contradictory reports about medications that cause cancer, or new deadly viral infections can be overwhelming. This is where your GP can help. “Doing everything in moderation” is my motto and taking a common-sense approach. If you are worried about things, speak to your doctor before making knee jerk decisions.

Our body is marvellously complex, but we still need to support and look after it. It’s always advisable to consider measures that help to support the daily functions it must perform to keep us alive. Early illnesses can often be detected before symptoms occur and medicals can help inform you about your health.

Here are some health tips for patients to prioritise their health and wellness in 2025:

1. Set Realistic Health Goals

- Start small with achievable goals like drinking more water or walking a set number of steps daily.
- Track your progress using wearable devices, apps or journals to stay motivated.

2. Keep Active

- Aim for at least 150 minutes of moderate exercise per week (e.g., brisk walking, cycling, swimming).
- Include strength training exercises at least twice a week.

3. Eat Unprocessed, Balanced Meals

- Focus on a diet rich in fruits, vegetables, whole grains, lean proteins, and healthy fats.
- Reduce sugar, salt, and processed foods. Check the ingredients – you’ll be surprised how the same foods can have completely different ingredients in the supermarket when you compare brands.
- Consider batch cooking and meal prepping to save time and ensure healthier choices are always to hand.

4. Prioritise Your Mental Health

- Practice mindfulness or meditation daily. There are a number of apps now that help guide you.
- Seek professional help if you’re feeling overwhelmed, anxious, or depressed.
- Stay connected with friends and family for emotional support.

5. Get Regular Checkups

- Schedule annual medical exams. An annual profile of blood tests can tell you a lot about your health.
- Attend routine screening appointments
- Keep vaccinations up to date.

6. Improve Sleep Habits

- Aim for 7-9 hours of quality sleep per night.
- Maintain a consistent sleep schedule, even on weekends.
- Avoid computer screens or mobile devices and heavy meals before bedtime.

7. Stay Hydrated

- Drink plenty of water throughout the day (aim for about 8 glasses).
- Limit caffeine and sugary drinks.

8. Quit Smoking and Limit Alcohol

- Seek support for smoking cessation through counselling or medications.
- Limit alcohol to 14 units a week over 4 days or avoid it altogether. There are some great 0% alternatives available now.

9. Manage Chronic Conditions

- Take medications as prescribed by your doctor.
- Monitor conditions like diabetes, hypertension, or asthma regularly.
- Consult your doctor for adjustments as needed.

11. Stay Informed

- Keep up with new health guidelines and advice from reliable sources like the NHS.
- Discuss health concerns with your doctor before trying new supplements, diets or fitness programmes.

12. Be Kind to Yourself

- None of us get it right all of the time!
- Celebrate small victories and don’t dwell on setbacks.
- Prioritise self-care to build a sustainable routine.

As the clocks go back and the days grow shorter, it’s easy to find yourself longing for those sun-drenched summer days. But aside from the nostalgic yearning for warm sunshine, the decrease in sunlight exposure during the winter months can significantly impact our vitamin D levels. This vital nutrient plays a crucial role in maintaining our overall well-being, making it essential to pay attention to its intake, especially during the darker days of winter in the UK.

First and foremost, vitamin D is crucial for our bone health. It helps our bodies absorb calcium, ensuring that our bones remain strong and healthy. In the absence of adequate vitamin D, our bones can become brittle, increasing the risk of fractures and osteoporosis. Checking your levels is as simple as taking a quick finger prick blood test from the comfort of your own home.

Don’t Let the Winter Blues Diminish Your Vitamin D: Why It’s Essential to Maintain Healthy Levels

Being from an Italian family and a lover of cars since I was a kid, I hope you’ll forgive me a short analogy.

After years of working in our beloved national health service, I realised that it was failing my patients. I adore the NHS, but I desired more focus on a preventative approach to healthcare. However, the system was not set up to work that way (it still isn’t).

I felt that the service was performing like a FIAT for my patients, but I wanted to be a Ferrari for them. That’s where the idea for Thyme was born. More on “why the name Thyme?” later.

There is a Healthspan crisis

As we live longer, we assume that we have more time to enjoy life. In reality, we’re spending a greater proportion of our life in ill health. The average lifespan since 1960 has increased by 19 years, but the percentage of our lives spent in good health has remained the same.

“For every extra year of life we have added to our life span, half of that may be in moderate or poor health. This ratio has been roughly constant for decades.”

Great health is about what we pursue:

I realised that most health services are reactionary and as a result we’re always chasing illness. There are providers of preventative screening around the world, but in the UK you pay a lot for a raft of tests. Next, you’ll get given a lovely, glossy report only to be told to see the very doctors that are too overworked to help.

Investing in your Healthspan for the long term:

Increasingly, it’s become apparent to me that as a population that we don’t invest in our health or our longevity enough. We just presume “it’ll be ok”. I doubt we’d take the same attitude with our investments, houses, pensions or holidays.

I wanted to move the healthcare narrative away from a reactive ‘sickness service’ to whole health care. I sought to create a healthcare service designed to enhance the portion of a person’s life spent in good health – their “Healthspan”.

A Whole Health approach:

The idea is simple. We need a health advocate and advisor to help us engage with sustainable changes to help us achieve our ‘best life’, long-term. The human body is a perfect machine, and all its functions are interlinked. To get the most out of it we need to eat well (fuel it), use it in the right way (exercise it) and look after it (service it). That’s the last car analogy, I promise!

It’s never too late to make positive changes to your lifestyle. Even later in life well informed changes can improve and extend your Healthspan.

As the human body and its functions are connected, we need an approach to healthcare that is also connected. An approach that incorporates preventative, lifestyle and traditional health services into one, managed package.
