I’ll be speaking generally about Dr Arun. I suffered from epilepsy from 2016 to 2019 visiting various doctors and hospitals. Their diagnoses were negative and the medication prescribed was incorrect due to these errors. However after seeing Dr Arun I’ve completely recovered from this illness and no more cramps. Oligodendrocytes and Myelin Plasticity Neurodegenerative diseases (NDs), including Alzheimer's disease, Parkinson's disease, amyotrophic lateral sclerosis/motor neuron disease, and multiple sclerosis, are characterized by progressive loss of neuronal structure and function, leading to severe cognitive, motor, and behavioral impairments. They pose a significant and growing challenge due to their rising prevalence and impact on global health systems. The societal and emotional toll on patients, caregivers, and healthcare infrastructures is considerable. While significant progress has been made in elucidating the pathological hallmarks of these disorders, the underlying cellular and molecular mechanisms remain incompletely understood. Increasing evidence implicates oligodendrocytes and their progenitors-oligodendrocyte progenitor cells (OPCs)-in the pathogenesis of several NDs, beyond their traditionally recognized role in demyelinating conditions such as MS. Oligodendrocytes are essential for axonal myelination, metabolic support, and neural circuit modulation in the central nervous system. Disruptions in oligodendrocyte function and myelin integrity-manifesting as demyelination, hypomyelination, or dysmyelination-have been associated with disease progression in various neurodegenerative contexts. Multiple sclerosis (MS) is a chronic, inflammatory, and immune-mediated disease of the central nervous system (CNS), primarily affecting young adults and often leading to life-long disability. Currently, about 28 disease-modifying treatments (DMTs) are approved for MS. Despite the availability of these highly effective therapies, some patients continue to experience highly active disease with an aggressive onset and a greater risk of long-term disability. Recent years have seen numerous retrospective studies, clinical trials, meta-analyses, and systematic reviews exploring autologous hematopoietic stem cell transplantation (AHSCT) as a potential treatment option to meet this clinical need. The findings indicate that AHSCT is a highly effective and relatively safe therapeutic approach for treating highly active MS. The body of evidence supporting AHSCT has expanded significantly, particularly in refining patient selection criteria, optimizing transplant techniques, and enhancing clinical experience. AHSCT represents the newest and most effective addition to the therapeutic options available for MS. There is a pressing need to raise global awareness about the applications of stem cell therapies. ALS, or amyotrophic lateral sclerosis, is a progressive neurodegenerative disease that impacts both upper and lower motor neurons responsible for movement, speech, swallowing, and breathing. ALS often starts with painless focal weakness, such as foot drop, hand weakness, or difficulties with speech and swallowing, and progressively spreads throughout the body. Over 60 genes have been linked to ALS, making genetic counseling and testing increasingly vital, especially as some therapies target specific mutations like SOD1. While a cure remains elusive, advancements in precision medicine, earlier diagnosis, and personalized care are on the horizon. Gene-targeted therapies, biomarkers, and coordinated care approaches are transforming our understanding of this disease. Six salient advances in understanding this devastating disease include: 1. ALS is not a single disease; genetics reveal multiple biological pathways involved. 2. Early symptoms can be subtle, often resembling orthopedic, spine, or speech-related issues. 3. Multidisciplinary care is crucial and can significantly improve survival and daily functioning. 4. Biomarkers like neurofilament light protein and TDP43 may help identify the disease earlier and track its progression more accurately. 5. The development of therapies such as tofersen demonstrates that targeting the underlying biology of ALS is feasible and may pave the way for future precision treatments. 6.
