Declan was amazing. From the Doctify reviews and his position, you know that he is top of his game and I found this very reassuring - no need to look further if you need RASP surgery. You don't get to be head of Urology at The Royal Marsden unless you're great. His team around him are excellent - Dr Netty in particular. They work extremely well together, and she has a little more time to indulge your inevitable questions. I was quite an 'inquisitive' patient, wanting to understand fully what I was getting myself into, in order to inform my decision: the risks and how I could mitigate them. His answers were reassuring without being glib, and he found time to answer me, despite a clearly heavy schedule. As it turns out, the operation and rehabilitation went swimmingly.

From my first consultation (online) with Declan I felt confident that I would be in good hands. In particular he was very positive that I would maintain continence which had been a concern. After what I’d heard from various sources beforehand the post-op recovery was very much better than I’d expected - I found the discomfort across the shoulders 24 hours later was the worst problem, pain from the abdominal wounds was minor and eased fairly rapidly after the first week. 3 months later I’m feeling 100% and am fully active. I had one unusual problem that meant I had to put up with the catheter for longer than expected but Declan and his team were fantastic and supportive throughout - it was disconcerting at the time but once sorted soon forgotten. I wouldn’t hesitate to recommend Declan if you need a prostatectomy, and have already recommended him to two friends who got tested because of my diagnosis.

Outstanding care from Mr Cahill and his team from initial consultation, surgery and after care. Mr Cahill explained my options and put me at ease immediately. I elected to have a da Vinci robotic prostatectomy which was performed at Guys. Declan is very down to earth and has a friendly, calm professional manner which is very reassuring. Most importantly in my experience he is an exceptional surgeon. I have recovered amazingly from the surgery. I regained my urinary continence within two weeks, he was able to perform a bilateral prostatectomy and now five months post op I have adequate sexual function which is continuing to improve. If you are considering surgery Declan is your man.

I was 46 years of age in November 2022 when I was diagnosed with prostate Cancer. I reached out to Declan's office the same day as my prostate cancer diagnosis. I was aware of Declan from a podcast where sports presenter Gaby Logan, and her Scottish rugby legend husband, Kenny, charted his cancer journey under Declan. From checking Declan out, It only took a bit of a Google to realise that Declan was close to or at the top of the list of prostate cancer surgeons in the UK. I had a first session with Declan via Zoom 2 weeks later. He was very kind and open to my wife and I, very understanding, answered all our questions and was also the right amount of blunt. We discussed options, I said I wanted to explore Nanoknife with Mark Emberton (another nice chap). Declan didn't feel this was the way I should go but was supportive. It turns out I was not suitable for NanoKnife, and we scheduled my surgery for January 24th 2023, almost 12 weeks after my diagnosis. In the run-up, I had sessions with Declan, but on the detailed side of the prep and the recovery, I mainly dealt with Dr Netty Kinsella. Netty is just a joy to deal with. She is patient, and understanding, and you feel in good hands. Whether it was before or after the surgery, I always felt like I could reach out to Netty, and I did. The night before my surgery, I was staying in a hotel close to the hospital and I missed a phone call from Declan. He left me a detailed message explaining how he and his team would look after me. He told me not to worry. I thought that was an amazing touch. Then I had my surgery. It went very well. My prostate was removed, and Declan explained that he was able to spare all of the nerves on one side and a majority (65%+) on the other. Great news. It was then all about recovery. In terms of pre and post-op, I did what Declan and the team said. I lost weight, I did loads of exercise and I got as fit as I could. Post-op, I took time off work, rested up, and slowly started walking around the block, and as soon as I was cleared, I hit the pelvic floor exercises, then went back to the gym, and took my meds (for erectile dysfunction). So What Are the Results? Cancer: I am cancer-free. Declan explained that in the post-analysis of my prostate, the cancer was all inside the prostate. I have had several PSAs since, and I am clear. Fingers crossed that remains the same. Continence: I have never needed or worn any daytime incontinence pads. I can pee when I want, and hold it if I need to. Between 24th Jan and 12th August 2023, there were 14 occasions, when sleeping after a few alcoholic drinks, I woke up in wet boxer shorts. My last episode was two months ago, and I am hopeful. Erections: I was able to get Erections post-op. I had several with the catheter in, not the most comfortable, but I was happy they were returning. As the weeks and months rolled on, I was concerned that they were not the same as before, and I could lose the erection, or find it difficult to summon. Both Netty and Declan kept saying that this was normal, and part of the journey. 8 months later, I can say that they were right. It really does get better with time. It's not 100%, but it's well on the way. I will keep working at it with the exercises, meds, and the VED. Size & Shape: The operation can change the size, length, girth, and even direction of your penis. That happened to me. Netty advised in pre-op prep that I should get a VED or Penis Gym. I got one but didn't start using it until 6 months later. It's a game-changer. I would recommend it to any man post-surgery, as soon as possible. Overall, I would recommend Declan, Netty and the team in London Bridge to anyone. If anyone is interested, I want to share my top tips and advice: POST BIOPSY/DIAGNOSIS & PRE OP ** Lose as much belly weight as possible pre-surgery (count the calories, I used my fitness pal, and I lost 10kg). ** Use the time between biopsy and operation for weight loss, Pelvic floor exercises, core exercises, and if possible weight training. ** But the bare minimum is weight loss and core exercises. ** I use the NHS Squeezy app for pelvic floor reminders and exercises. I still use it today. ** I found the surgery easier and less uncomfortable than the Biopsy. The surgery is not painful. Recovery ** When you are at home, you will have a catheter (tube out of your penis) in place, and 6 patched-up wounds in the tummy - sleep on your own if possible ** Recommence the pelvic floor as you are allowed ** Get A VED or Penis Gym and use it AS SOON AS THE DOCTER SAYS YOU CAN. The operation can change the size, length, girth, and even direction of your penis. The Vacuum Erection Device is easy to use and REALLY makes a difference. Before I started to use it, those changes were getting me down. Things are much better now. ** Get back to exercise as soon as you are allowed - do core work. ** I had several sessions of scar tissue massage after week 6 post-op. That really helped with mobility and scar healing. You cannot believe how your mobility can be affected by tightness in and around the 6 scars. ** I had some episodes of incontinence at night time, I felt embarrassed, and I didn't handle it that well. My advice is don't be embarrassed, use the pads, it will get better. I am now 8 months post-op, and I have had no nighttime accidents in the last 2 months. ** I have never needed or worn any daytime incontinence pads. ** I reached out to a local Prostate cancer support group, as I was having a bit of a hard time with the changes in my penis, my erections and my nighttime incontinence episodes, and the whole Cancer thing. It got on top of me and I didn't realise until one night over a beer it boiled over. The group recommended a McMillan initiative through Bupa, which was 6 free counseling sessions. They really, really helped me. ** I also asked Declan if I could speak to one of his patients born the same year as I was. He did, and that also very much helped me. The more you can talk about it, and listen to others with the same challenges, the better.
