In February 2025 I underwent private refractive lens exchange (RLE) with Shafiq Rehman at Optegra. My cataracts were mild and not materially affecting my daily functioning. The primary goal was refractive — to reduce dependence on glasses. After consultation, I selected toric lenses rather than multifocal lenses. I was advised they would provide strong distance and intermediate vision with a lower incidence of glare. Twelve months post-operatively, my visual quality and ocular comfort are significantly worse than before surgery. Ongoing issues include: • Severe chronic dry eye • Persistent glare affecting night driving • Reduced visual clarity and clouding • Symptomatic floaters interfering with central vision • Posterior Capsular Opacification (PCO) Although I can achieve acceptable results on standard visual acuity testing and the lenses are reportedly well positioned, my functional visual experience is substantially impaired. I acknowledge that risks are documented in the patient information material. However, in my personal experience, the potential severity and long-term quality-of-life impact of dry eye and vitreous symptoms — particularly in high myopia — were not emphasised in a way that reflected the consequences I now face. I have been offered YAG capsulotomy for PCO, but understand that this can increase vitreous debris in some patients, so I am currently reluctant to proceed. Any treatment of vitreous symptoms may fall outside Optegra’s aftercare arrangements and could instead require treatment elsewhere, potentially within the NHS. This highlights a key consideration in relation to care pathways. Optegra provides both NHS and private cataract surgery. In my case, the procedure was private and subject to a defined aftercare period of 12 months. Patients should understand what arrangements would apply if significant longer-term complications arise beyond that period, including whether ongoing treatment would fall within the private provider’s remit or elsewhere. This outcome has had a profound impact on my daily life, including driving, screen use and sport. Had I fully appreciated the potential long-term consequences in practical terms, I would not have chosen elective RLE at that stage. While many patients may have excellent outcomes, my experience has been deeply negative.

Overall it was a good experience, apart from the anaesthetist. It took 6 tries to put the cannula in various places, the young lady was very kind and tried to be gentle but it was very painful. Finally we thought the cannula was in and drugs administered, but not. I’ve had my hip replaced under epidural and remember that there was only one injection which was mainly painless. I ended up having FIVE spinal injections, each extremely painful. Finally my lower limbs were numb and I was relocated to the operating theatre. Once there the drug to make me sleep was administered and I had terrific pain in my bicep above the cannula. Immediately it was relocated and I fell asleep. To the best of my knowledge, the operation went well and the surgeon was pleased with the outcome. Most of the staff were very friendly, helpful and knowledgeable. Unfortunately a young man on night duty didn’t seem to have a clue about bed pans when recovering from an epidural. I was mortified that my gown and the bed protector had been caught up in the pan resulting in everything getting wet. I felt a lack of dignity needing to have myself and the bedding changed. He was a nice young man but not very experienced. If it weren’t for the epidural incident, I wouldn’t hesitate to use Horder when I need my other knee done. However, I think I will need to leave it as long as possible so that the memory fades. I would still recommend anyone to use your facilities.

I had to have an MRI and sigmoidoscopy. The MRI was fine, the booking procedure admin hadn’t mentioned the fact I would need an IV and as someone hard to find veins particularly first thing in the morning when dehydrated this was difficult and stressful. They acknowledged and apologised but this part of the process could definately be improved. The rest of the MRI procedure went very well. Mr Sains appointments and care was exemplary and he made me feel at ease able to discuss difficult symptoms clearly and carefully. The hospital admission was excellent and I was very well looked after and comfortable and very thankful to be at the montefiore and not elsewhere in the city. The procedure itself went well and the nursing care and support during a stressful procedure second to none. I would not hesitate to recommend this hospital, its care and Mr Sains.

Everyone (without exception) has been extremely professional, knowledgeable and very efficient but what is most important, when you are going through something that is somewhat unsettling, they were all extremely caring and kind. Parking at the William Harvey Hospital Ashford is poor. There was a long queue of cars circulating, hunting for non-existent spaces. There is a microscopic Spencer Wing car park for patients and staff, but that is routinely filled around 8 a.m. and is not available for patients at all. I arrived 15 minutes early for my appointment, but it tool an hour to complete the treatment. Early morning appiontments normally go swiftly and on time. Afternoon appointments tend to back up.

I was in a lot of pain and really needed an urgent scan of my lower back. The radiographer was so kind and understanding. After the scan, he recommended I go straight to A&E because he had seen something that required urgent medical attention. They sent me a link to my MRI images immediately, which I showed the doctors in A&E. The doctor also spoke to the radiographer whilst I was there. I’m really grateful for how supportive and helpful they were during a stressful time.

Mr cohen is really brilliant. Eight months in from my hip replacement I wouldn't know the hip isn't mine and the scar which is barely 6 inches long is hardly visible. Mr Cohens bedside manner could do with some work, but I went to him for his expertise and wasn’t disappointed. I wasn’t looking for kind words, I was looking to be pain free, and he delivered thst in spades.
